Frequently Asked Questions

How soon would I be able to use my plan?

For Dental: Eligibility date is about 30-45 days after payroll deduction is processed. We must receive two payments for eligibility. Eligibility is established on the 1st of the month following the 2nd payment.  (Example:  Your deduction is made on the 1st payroll of January, your eligibility would be on the 1st of February, if your deduction is made on the 2nd payroll in January your eligibility would be the 1st of March).

For Vision: Eligibility is established on the 1st of the following month if we receive the allotment before the 15th of the month otherwise eligibility will be set for the following month. (example: if allotment is recieved before January 15, you eligibility will begin on February 1st; if your allotment is received the 16th or later of January your eligibility will begin March 1st)

Do I need to choose a dental and vision provider?

You need to choose a Dental Provider because the Dental is a HMO and you will be assigned to a dental office or facility. If you do not choose a Dental Provider you will not be assigned to a dental office and you will not receive a membership card. The Vision is a PPO so you can go to any Vision Provider but there is corresponding reimbursement percentage if you will choose a In Network or Out of Network Providers (please refer to the Summary of Benefits)

When will I receive my membership card?

Your membership card will be mailed out to you before your eligibility starts for Dental and Vision. You do not need an ID Card for Dental to show to your dentist as a proof that you have coverage. Just call your dentist or facility to schedule an appointment after your effective date.

What will happen after you receive my enrollment form?

We will call you to help you to process the allotment. It is ultimately your responsibility to start your payroll deduction.

What about dependents coverage? Do you have restrictions on who to cover or not?

We do cover children until the age of 26, they do not need to be full time students. We can only cover legal dependents; therefore you must provide proof of guardianship for grandchildren. You may pay for your parents’ enrollment.

Do I need to renew my dental or vision every year?

You do not need to renew your plan every year so long as we receive your allotment you are covered.

What if I need to see a specialist?

Your selected dentist or facility will refer you to a participating specialist in your area and any co-payment amount is listed on your Schedule of Covered Services.

Can I change a dentist?

Yes you can. You may call us at 1-888-647-8777 and you need to complete a change form to make the transfer and it will be effective on the first of the following month.

How do I change my coverage?

You can add a dependent at anytime. You can drop dependents due to a qualifying life change event call us at 1-888-647-8777 for more information and/or to complete the appropriate form.  Note: a one year minimum may apply if Dental services have been used. 

Can I cancel my coverage?

After one year from the start of your policy.

Can I be dropped from my Dental Coverage?

No.
